How Online Scheduling Works
To make scheduling easier, Cornerstone Medical Center offers secure online appointment scheduling. Follow the steps below to request your appointment.
Step 1:
- Review the available appointment times shown on the scheduling page.
- Select a date and time that works best for you.
- Each visit is 30 minutes.
Step 2:
- Fill in all required fields, including:
- Patient name
- Date of birth
- Email address
- Phone number
- Reason for visit
- All fields are required to submit your request.
Step 3:
- Click the “Submit Appointment Request” button once the form is complete.
Step 4:
- You will receive an email confirmation after submitting your request.
- Open the email and click the confirmation link to finalize your appointment.
- Your appointment is not scheduled until you complete this step.
Step 5:
- Once confirmed, your appointment is officially scheduled with Cornerstone Medical Center.
